2018 DJFL/FCFL Football Season Agreement and Questionnaire (each parent/guardian must sign). If you register online, all forms are already INCLUDED in your registration. This is a seven-page document, which includes a Parent Code of Conduct and Participant Code of Conduct form as well as Waiver and Permission to Treat forms.
$400 equipment deposit check payable to “DJFL” and post dated 12/1/2018--(required for TACKLE). For Cheer, a $150 equipment deposit check payable to “DJCL” and post dated 12/1/2018 is required.
